Job Summary
Heart failure doesn't mind your age or what you look like... and it's on the rise. This role offers an opportunity to be part of a dynamic team exploring ways of supporting our patients living with heart failure. The post will help expand and develop services across the Vale of York, including acute and community settings, with involvement at the front of an innovative service and opportunities to develop knowledge and skills in heart failure while working with committed colleagues. Support will be provided to work towards the heart failure specialist nurse competency framework set by the British Society of Heart Failure.
Main duties of the job
The post holder will be part of a team of experienced heart failure specialist nurses and will provide high‑quality expert care to patients with heart failure in York hospital and the Vale of York community. The heart failure specialist nurse team delivers an out‑of‑hospital rapid access heart failure clinic, community clinics, and home visiting to housebound patients. The team is also building a heart failure in‑reach model focusing on admission avoidance and early step‑down to community‑delivered care.

* Registered nurse
* Degree-level specialist training or equivalent cardiac experience
* Non-medical prescribing qualification
