Are you a Nurse Practitioner ready to transform lives and make a real difference for patients with complex kidney conditions? We are looking for a passionate NP to join our team in Bakersfield, helping patients navigate a challenging healthcare system while receiving holistic, integrated care.
Nightingale Clinical Care, PC is an innovative team of passionate professionals who tailor care plans for patients with complex needs, and work closely with all care team providers with the goals of cultivating a patient centered care approach, improving care consistency, and enhancing the coordination of patient services.
As a Nightingale Nurse Practitioner you will support some of the most complex patients, assisting them in navigating a challenging healthcare system, and providing the one-on one-patient care they need and deserve.
Nightingale Nurse Practitioners will provide patient education, create dedicated care plans, and provide clinical guidance to an integrated care team all with the best interest of the patient as the guiding principle.
This Nurse Practitioner will work closely with DaVita’s Integrated Kidney Care team to provide superior care for our dialysis patient population.
This Nurse Practitioner will function autonomously, and in collaboration with all members of the healthcare team to coordinate and facilitate high quality, cost-effective care. This Nurse Practitioner will provide coordinated care services between the patient, family, physicians, additional providers, and other care team members.

Position Details:
1. Location: Hybrid – Remote. Occasional work from home (telehealth) with travel across an assigned geographic area, including dialysis clinics and nephrology practices. Travel expectations may vary based on business needs and patient population.
2. Clinical Care & Evaluation: The primary responsibility is completing Comprehensive Health Evaluations (CHEs) to assess and manage comorbid conditions while addressing medical, social, emotional, and financial needs. Reduce hospitalizations and improve patient outcomes.
3. Care Management & Disease Progression: Manage CKD and ESKD patients, focusing on slowing disease progression, reducing costs, and preventing readmissions. Utilize prescriptive authority, medication management, and diagnostic interpretation in collaboration with nephrologists and interdisciplinary teams.
4. Clinical Leadership: Lead the clinical team to ensure care coordination aligns with medical treatment plans and addresses both medical and psychosocial needs.

Requirements
6. Passion for caring for patients with complex, chronic illnesses
7. Willingness to adapt as the program evolves
8. Minimum 2 years’ experience as a Nurse Practitioner (NP)
9. Current, unrestricted NP license in state of practice; DEA license or eligibility within 90 days
10. Current CPR/BLS certification
11. Valid driver’s license and insurance in the state
12. Intermediate computer skills (MS Word, Excel, Outlook)
13. Strong clinical judgment, autonomous decision-making, and expertise in managing complex comorbidities
14. Positive, solution-focused attitude, committed to improving patient and team outcomes
15. Home office, high-speed internet, and reliable transportation
16. A valid driver’s license – NCC Nurse Practitioners typically need to drive their own automobiles to get from one location to another. You’ll need a valid driver’s license within the state you are applying for work, as well as valid automobile insurance and an acceptable driving record.
Preferred Qualifications:
17. Experience in Value-Based Care or Population Health, including Medicare Advantage documentation and Health Risk Assessments
18. 5+ years’ experience as a Family NP, Primary Care NP, or in Internal Medicine, Cardiology, Nephrology, or Endocrinology.
19. Experience managing population health with a focus on reducing hospitalizations
20. Prior experience with Cerner or similar EHR systems

Salary Range:
$130,000-$155,000
