About the role
As the Falls Practitioner Physiotherapist for the Wirral Falls Prevention Service you will provide a high‑quality service to pre‑frail older adults who have fallen or are at risk of falls. Through multifactorial falls risk assessments and by providing Public Health advice and interventions to promote an active and healthy lifestyle in order to prevent falls.
Responsibilities
* Undertake a specialised comprehensive multifactorial falls risk assessment and provide an accurate diagnosis or impression of falls risk factors.
* Formulate intervention plans and recommend the best course of action.
* Assess and provide low‑level community equipment and walking aids, including grab rails; refer more complex cases to specialised therapy services.
* Prescribe low‑level exercises for strength and balance and support participation in community exercise programmes when required.
* Refer patients to other agencies and professionals.
* Work within the Chartered Society of Physiotherapy (CSP) guidelines and maintain a good working knowledge of national and local standards, monitoring own quality of practice.
* Maintain and develop current knowledge of evidence‑based practice.
* Maintain accurate and comprehensive clinical records and organise and plan caseload to meet service and patient priorities, readjusting plans as situations change or arise.
* Assist in training and development and supervise technical instructors and students.
* Ensure that your own practice and that of staff under your supervision meet professional standards of practice.
* Contribute to the Clinical Governance agenda and participate in audits as required by the Service.
* Deputise for the Team Leader in their absence, taking responsibility for operational management of the team.
Qualifications and experience required
* Diploma / Degree in Physiotherapy.
* HCPC registered.
* Full UK Driving Licence and access to own vehicle for work purposes.
* Band 6 equivalent experience is preferred.
