Job Description American Traveler is seeking an experienced RN Case Manager with DC licensure and case management experience for a weekday, office-based assignment.
Responsibilities In-office case management role with some required face-to-face assessments at a wellness center in Washington, DC
Population includes enrollees with chronic medical and behavioral health diagnoses such as high-risk pregnancy, diabetes, CHF, asthma, and obesity
Work schedule is Monday through Friday, 8:00 am to 4:30 pm with no on-call or weekend requirements
Case managers make calls to enrollees, complete comprehensive assessments, and manage ongoing care plans using the EMR system (Guiding Care/HealthEdge)
Business casual dress code is required
Responsibilities include completing assessments, developing care plans, and ongoing management for members with chronic health issues
Chronic disease and behavioral health case management are key focuses of this position
Flu vaccination is required for all employees
Potential contact with COVID patients is possible
Training is provided in the office setting
Modules are required prior to start and during orientation, tracked via transcripts
Candidates who have previously worked for MedStar must have a 1-year gap before returning
Local candidates within 60 miles are accepted but at a local rate
A maximum of 7 days of RTO per 13-week contract is preferred, with no RTO allowed in the first two weeks
Requirements Active DC RN license required prior to start; pending license accepted if obtained before assignment begins
Current Basic Life Support (BLS) certification required
At least 1 year of relevant case management or care coordination experience is required; 2 years preferred
Experience with acute hospital, LTAC/rehab/SNF, and discharge planning highly preferred
Experience utilizing InterQual Criteria and Milliman Guidelines required
Knowledge of CPT codes, DRG, HEDIS, NCQA, OSHA, and JCC regulations required
Charge experience preferred
EMR experience, particularly with guiding care/HealthEdge, preferred
First-time travelers are welcome if possessing a strong background
