Main Duties
* Support patients with personal care, ensuring dignity, privacy, and respect at all times
* Assist with meals and encourage good nutrition and hydration
* Promote independence where possible while providing appropriate support
Monitoring and Observations
* Carry out and accurately record regular physical and mental health observations, including mood, behaviour, and vital signs
* Promptly report any changes or concerns to the nursing or medical team
Promoting Recovery and Engagement
* Encourage patient participation in therapeutic, social, and recreational activities
* Contribute to a positive, recovery-focused ward environment
Communication and Relationship Building
* Build and maintain professional, therapeutic relationships with patients
* Engage in meaningful conversations to provide emotional reassurance and support
Maintaining a Safe Environment
* Support the safety and security of the ward in line with observation levels and trust policies
* Assist in managing patient distress or crisis situations, including the use of de‑escalation techniques
Documentation and Record Keeping
* Accurately document care provided, observations, and incidents using electronic record systems
* Ensure all documentation complies with trust policies, confidentiality, and information governance standards
Teamwork and Professional Development
* Work collaboratively within the multidisciplinary team, including nurses, doctors, occupational therapists, and psychologists
