About The Opportunity

The Care Partner Support Officer provides administrative and systems-based support to the Care Partner team, assisting with documentation, scheduling, and data entry to ensure timely and effective delivery of care planning and coordination services.


The day-to-day responsibilities

* Triage, resolve, or escalate all phone and email enquiries to ensure timely and satisfactory outcomes for clients and their representatives.
* Provide administrative support to Care Partner teams, including preparing and sending referrals, forms, correspondence, and care documentation.
* Provide clients and their representatives accurate, high-quality information to support informed choices about care.
* Uphold confidentiality and maintain data integrity across all administrative tasks.
* Contribute to service reporting, audits, and continuous improvement initiatives.
* Adhere to all Fronditha Care’s policies, procedures.
* Maintain respectful communication and professional boundaries with clients, families, and colleagues.
* Assist in the development and coordination of Level 1 and Level 2 care plans under the guidance of Senior Care Partners.
* Maintain accurate records and case notes in client and service management systems.
* Record and report incidents within required timeframes using the incident management system.
* Promote Fronditha Care’s services within the community to enhance visibility and access.
* Support a safe and healthy workplace by following safety procedures and reporting hazards or injuries.


What success looks like for this role

* At least 2 years’ experience in aged care, or formal qualifications such as a Certificate in Community Services or a related field.
* Tertiary qualifications in a health-related or allied health field (preferred)
* Excellent PC skills including experience with Microsoft Office and databases.
* Familiarity with aged care systems, care planning or aged care processes.
* Strong administrative and organisational skills with high attention to detail
* Excellent written and verbal communication skills
* High interpersonal and problem-solving skills
* Ability to manage competing priorities and meet agreed timelines
* Ability to work independently and collaboratively within a team
* Commitment to the rights, dignity, and inclusion of older people
* Manage a portfolio of clients, including their care needs and funding
* Work within Fronditha Care’s Consumer Directed Care Model of Service.
* Ensure service delivery meets consumers’ needs, Home Care Package Operational Manual, The Aged Care Act 1997 and Fronditha Care’s quality assurance goals.
* Great knowledge around Home Care Packages and Funding Levels.
* Have a 'can-do' attitude
