An exciting opportunity has arisen for an individual to work within mental health urgent care as a Discharge Coordinator within the Crisis Resolution and Home Treatment teams across Liverpool and Sefton geographical footprint.
The post holder will act as a key liaison between inpatient units, the crisis resolution home treatment team, and the bed management team. They will be responsible for the effective coordination of patient discharges throughout the mental health inpatient units. The post holder will work closely with inpatient clinicians and multidisciplinary team members to deliver a high-quality, patient-focused service.
The post holder will be based with the CRHT at Broadoak Unit but will be expected to mobilize between sites to monitor capacity and patient flow through CRHTs and inpatients.
They will attend and participate in daily bed capacity meetings, providing predicted discharge activity for the day and data related to discharge planning progress.
The role involves liaising with colleagues such as CRHT, ward staff, and community services to ensure seamless discharge processes.
The post holder will inform staff when inpatient capacity may be insufficient to meet demand and will request additional support to expedite discharges.
They will arrange and attend discharge planning meetings on allocated wards and participate in daily multidisciplinary morning meetings to monitor and adhere to estimated discharge dates.
Follow-up reviews post-discharge with service users in the community will also be part of the role.
