The Provide Stroke Rehabilitation Service supports 25 inpatient Stroke beds and a well established Early Supported Discharge Service. We work in partnership with the Acute Stroke Units, Social Care and the Stroke Association providing seamless, high quality stroke rehabilitation to the people in our care. Our Nursing and Therapy teams, supported by Stroke Psychologists and Medical Practitioners provide a progressive service which showcase true multi and interdisciplinary working. Our philosophy encourages staff participation at all levels and identifies opportunities for service and career development.
This is an exciting opportunity to join a team of stroke skilled professionals going through a development process of increasing beds and partnership working with MSEFT to create a seamless end to end pathway for stroke patients. The post holder will be working across ESD and the Stroke Rehabilitation Unit in Brentwood, giving an excellent opportunity to work across the community pathway, focusing on the delivery of the ICSS model 2022.
