Personal Service Coordinator (Temporary)

35 hours per week / $24.00 per hour

(The actual compensation will be determined based on experience and other factors permitted by law.)

Summary: Under the direct supervision of the program manager, utilizes evidence-based outreach and engagement, harm reduction, housing first, motivational interviewing, and supported employment and education practices to support members. Integrates primary care and mental health care services within a mobile, homeless outreach, and intensive case management team model. Assists participants in achieving recovery, living in permanent housing, and becoming full community participants based on psychosocial rehabilitation models.

Essential Duties and Responsibilities: This is not an exhaustive list, and other duties may be assigned. * Collaborate within an integrated mobile team to provide street outreach, community-based case management, and linkage to permanent housing for homeless individuals.
* Conduct outreach and engagement efforts with homeless individuals through intensive, sustained, and consistent processes.
* Develop and complete mental health recovery plans with members, identifying personal goals and needs.
* Support members in securing independent housing by overcoming barriers, accessing funding, educating on tenants’ rights, responding to housing concerns, conducting home visits, and collaborating with property managers and partners to retain housing.
* Provide transportation to services and accompany individuals on public transportation to ensure access.
* Offer life skills training to enhance daily living skills such as money management, hygiene, meal prep, transportation, household management, social activities, and employment/educational goals.
* Mentor, advocate, and educate members within scope of practice to reach recovery goals.
* Participate in ongoing training to learn new interventions and strategies.
* Advocate for members to obtain and maintain housing, healthcare benefits, and entitlements.
* Maintain accurate records and documentation using technological resources for compliance and accountability.
* Meet documentation and billing requirements, including timely note entry and corrections.
* Participate actively in team meetings, communicating proactively about member concerns.

Position Requirements:

* High School Diploma or GED with at least one year of experience providing outreach and psychosocial rehabilitation services to homeless or disabled populations.
* Bachelor’s degree in a human services or related field preferred.
* Bi-lingual skills preferred.
* Experience or lived mental health experience preferred.
* Valid Class C driver’s license, vehicle, proof of insurance, and ability to be insured by MHA are required.
* TB clearance and annual re-testing required.
* Flexible schedule including some evenings or weekends; on-call cell phone availability.
* Valid CPR and First Aid certification.
* Ability to provide services in the field at various locations.
* Culturally and linguistically responsive intervention skills.
* Empathy and creativity to overcome barriers faced by individuals in severe need.
* Understanding of community resources and evidence-based practices such as motivational interviewing, harm reduction, supported employment, and housing first.
* Ability to work effectively in a multidisciplinary team in a fast-paced environment.
* Knowledge of substance abuse and dual diagnosis characteristics.
* Proficiency in Microsoft Word, Excel, and Outlook.
