Responsibilities
Within Locala’s Health and Wellbeing Integrated Communities, we deliver a range of multidisciplinary community services, including integrated health and social care in collaboration with Kirklees Council. Locala has successfully implemented service improvements and planning, providing enhanced support to patients, improving their experience, and ensuring excellent continuity of care.
The health care support worker role offers a great opportunity to develop rehabilitation skills. The role involves delegated nursing and therapy tasks, such as:
* Daily mobility and transfer practice with patients
* Facilitation of group exercise programs
* Basic nursing tasks including pressure ulcer prevention and malnutrition care plans

The service is based at Ings Grove House, serving North and South Kirklees residents. There may be a need to work within patients' homes across Kirklees to support home visits and outreach services.
We operate a 7-day service, working 8 am to 7 pm in a rota pattern, providing assessment, treatment, and evaluation of clients’ needs using a holistic, patient-centered approach. The goal is to prevent unnecessary hospital admissions and facilitate swift, appropriate discharges to support rehabilitation and return home.
Initiative is essential, with opportunities to support and share skills within the team and across sectors. This is an exciting time to join a well-established team with ongoing service transformation projects. Regular clinical and managerial supervision will be provided, along with opportunities to develop clinical skills.
