Adult’s Social Worker, Immediate Care Team, Portsmouth Pay rate £32 to £35 per hour Contract role, Community Team Pertemps are recruiting for an experienced Adult’s Social Worker in Portsmouth. This is to work in a Immediate Care Team Your role will involve assessing the needs of adults and their Carers, to identify creative ways to meet their assessed outcomes under the Care Act 2014. We are looking for a social worker to work across all of these areas, so would be the ideal opportunity for someone seeking a variety within their work. One day you may be working with a person in hospital and supporting them with their discharge planning, ensuring a home first approach at each stage, the next day, visiting a person in their own home, supporting them back to independence through the application of the strengths -based approach. The role includes outcome led interventions, using a strength-based approach to support people to live the lives they chose. You will work collaboratively with others, maintaining a strength based and aspirational approach to achieve the outcomes the person wants. Duties include assessing clients’ needs and agreeing a person-centred support plan, completing reviews of assessments and support plans, supporting people with their health, mental health, and social care needs, and assessing mental capacity when required.
