Overview
This fixed term post is to increase External Out of Area (OOA) Provider visits and drive down Length of Stay, Delayed Transfer of Care and link with medics in terms of monitor/challenging level of observations; with the objective to lead to improved patient flow. You will be required to coordinate admissions for adult Acute patients signpost Frail functional patients, and patients diagnosed with Dementia. You will have designated responsibilities for at least one area of expertise, Adult Acute or Old Age. You will particularly focus on discharge delays arising out of health and social care needs e.g. accommodation, benefits, Home Office and asylum seeker issues - to include the identification of any overseas visitor in relation to charging for NHS care and liaise with the Enhanced Discharge Team to create capacity so as to manage flow effectively.
The new Enhanced Discharge Team under Integrated Patient Flow Service is looking for an enthusiastic, dynamic person who is interested in working as part of a multi‑disciplinary team in managing patient flow through planned, strategic admissions and discharges. The post holder must be an excellent communicator who demonstrates good clinical and organisational skills and be able to reassure carers, referrers and senior management over disputed and distressed admissions/discharges. To communicate with the Senior Management Team, senior clinicians, external agencies, staff, service users and carers on an individual case basis and also the broader issues around both emergency admissions and discharge arrangements. This will require persuasive negotiating skills and the ability to work at an individual case level as well as presenting a more strategic approach both internally and externally. You should be a visible and proactive clinical leader and role model within a multi‑disciplinary framework.
Key Requirements
* Nurse, Occupational Therapist or Social Worker
* Relevant degree or equivalent post qualification training
* Evidence of professional registration (NMC/HCPC/SWE)
Desirable Criteria
* Management Qualification or equivalent management experience to masters level
* Approved Mental Health Professional
* Some senior level experience at band 7, Strong Leadership and Motivational Skills
* Ability to prioritise tasks and to meet deadlines.
* Ability to consider more than one solution to a potential problem, Knowledge and awareness of good practice in relation to in‑patient admission and discharge procedures
* Knowledge of legislation and practice relevant to the care group, Experience of working with people with mental health problems and working in a care environment
* Demonstrable clinical skills, including assessment and care planning within CPA
* Experience in extra contractual referrals (ECRs) and Individual Patient Agreements (IPAs)
* You must have appropriate UK professional registration.
