Job Overview
An exciting opportunity has arisen for a dynamic and enthusiastic individual to join the Mental Health & Learning Disabilities Commissioning Team within Aneurin Bevan Health Board as a Case Manager. The MHLD Commissioning Team oversees the arrangements for individuals who are commissioned packages of care.
Main Duties
The post holder will be responsible for the provision of seamless, integrated services and packages of care for individuals who meet Continuing NHS Healthcare criteria. They will play a key role in the implementation of the National Framework for Continuing NHS Healthcare within the Mental Health & Learning Disabilities Division, and will provide a case management function to a caseload of patients or service users eligible for Continuing NHS Healthcare or subject to the 117 that require commissioned services that cannot be met by core services. The post holder will support the development and implementation of governance arrangements to improve the quality of care or services delivered to the local population, regardless of setting, and apply financial due diligence when commissioning packages of care.

The Agenda for Change Pay Award for 2026/27 is effective from 1 April 2026. Successful applicants with a start date from 1 April onwards will receive a salary as per the 2026/27 pay award. Essential Criteria
* Masters level or equivalent demonstrable level of knowledge and experience
* RMN/RNLD qualification
* Working knowledge of the complexities involved with Complex Care Packages
Desirable Criteria
* Experience of planning and commissioning within Continuing NHS Healthcare/Complex Care settings
