36 hours per week, 52.14 weeks per year, Permanent The successful candidate will be based within the Integrated Discharge Hub - Hospital Social Work Team and responsible for completing assessments for adults and older people to facilitate safe and supported discharges from hospital. The Integrated Discharge hub is an inter agency multi disciplinary team made up of social workers, community care officers, reablement staff, discharge facilitators, AHP staff as well as other representatives from different services. The hub promotes a home first approach and works towards patients being discharged on their predicted date of discharge (PDD). This is an exciting opportunity for the right person to work with a range of professionals and people to support the principles of Home First. The worker must be able to follow policies and procedures, have excellent communication skills in addition to developed assessment and report writing skills. The worker will be required to provide daily updates on their caseload in relation to discharge planning. Tasks will be carried out in partnership with service users, family members, colleagues within Social Policy and other professionals and agencies as necessary to meet need.
