Based at St John's Information and Support Centre with frequent time spent at Doncaster Royal Infirmary and GP practices across Doncaster.
An exciting opportunity has arisen for a highly motivated, enthusiastic individual to join an existing team to work across Doncaster providing support to people living with cancer in the community, including a focus on 'hard-to-reach' groups.
The postholder will be responsible for conducting holistic needs assessments contributing to the Personalised Care and Support Plan, ensuring people's physical, practical, emotional & social needs are identified and addressed at the earliest opportunity, referring and signposting to supportive services.
The post holder will demonstrate excellent organisational skills, must be flexible in their approach, able to exercise initiative & demonstrate a consistently high standard of professionalism. The post holder will have excellent communication skills & be willing to undertake ongoing training and development as part of their orientation.
Applicants should have significant experience of working in healthcare or similar public facing role.
A major remit of the post involves supporting patients with cancer and their families, both face to face and via telephone, who are referred by the clinical teams as well as those patients/relatives seeking info, advice and support independently. The role will use an existing framework to holistically assess patients to identify their information and support needs.
Main Duties of the Job
The postholder will focus on providing high-quality patient care & support by managing a caseload of patients in different settings with a cancer diagnosis, working across a PCN footprint. As a key point of contact, the postholder will be responsible for conducting holistic needs assessments contributing to the Personalised Care and Support Plan, ensuring people's physical, practical, emotional, and social needs are identified and addressed at the earliest opportunity, referring & signposting to supportive services, working alongside practice staff as an extension of the Macmillan Cancer Information and Support Service. This diverse & key role will involve working across a number of GP practices in Doncaster and establishing close working relationships with acute Trust staff at Doncaster & Bassetlaw Teaching Hospitals NHSFT as part of the Macmillan Cancer Information and Support integrated pathway.
In addition, the team work across the Doncaster community with public health partners and local organisations and groups to promote awareness of the service offer and increase accessibility. The service also focuses routinely on delivery of cancer health promotion messages relating to early signs and symptoms, importance of early diagnosis, cancer screening and reducing cancer risk.
