Care Coordinator – Supported Living Services
Location: Reading
Salary: £26,000-£32,000
Hours: Full-time, Permanent Monday to Friday
Supported Living (Adults with Learning Disabilities, Autism, Mental Health Needs)
Fantastic opportunity for a candidate with strong administration and communication skills, who has a background of hands of support and rota organisation.
Monday to Friday to work alongside the residential manager to co-ordinate staff for 2 supported living services and 1 outreach client.

We are currently seeking a Care Coordinator on a permanent basis to join one of our clients who have 2 supported living services in the Berkshire area and are looking at expanding.
Key Responsibilities:
Work alongside the manager to coordinate and schedule care and support for service users across supported living settings.
Work closely with the Registered Manager to ensure care plans are up to date and effectively implemented.
Monitor and maintain accurate records, including risk assessments and support plans.
Support with staff rotas, shift planning, and responding to last-minute cover requirements.
Be a key point of contact for staff, service users, families, and external professionals.
Ensure all support is delivered in line with CQC standards, safeguarding policies, and internal procedures.
To be included in on calls and able to step in should their be a need.
Requirements:
Previous experience in a care or support setting (supported living or domiciliary care preferred).
Experience with rota’s and shift coordination preferred.
Strong communication, organisational, and problem-solving skills.
Good understanding of person-centred care and safeguarding.
Proficient in using care planning systems and general IT.
A caring attitude and commitment to empowering individuals to live independently.
NVQ Level 3 in Health & Social Care (or working towards) is desirable.
