We are seeking a dedicated and compassionate Qualified Social Worker (QSW) to join our Older Adults Community Mental Health Team (CMHT) in Bolton. This role is pivotal in providing high‑quality support to older adults facing mental health challenges. As a QSW, you will work collaboratively with service users, their families, and multidisciplinary teams to assess needs, develop care plans, and implement effective interventions.
Your key responsibilities will include:
* Conducting comprehensive assessments of older adults’ mental health needs.
* Developing, implementing, and reviewing care plans in partnership with service users and their families.
* Providing direct support and therapeutic interventions to enhance the well‑being of older adults.
* Collaborating with healthcare professionals, community organizations, and other stakeholders to ensure holistic care.
* Maintaining accurate and timely records in accordance with relevant policies and procedures.
* Participating in supervision and professional development activities to enhance your skills and knowledge.
To be successful in this role, you will need to possess the following essential qualifications and skills:
* A recognised social work qualification (e.g., BA/MA in Social Work).
* Current registration with Social Work England.
* Experience working with older adults and understanding of mental health issues.
* Strong assessment and analytical skills.
* Excellent communication and interpersonal abilities.
