Overview

Experienced General Practice Nurse (Frailty) – The Confederation, Hillingdon CIC

The Confederation, Hillingdon CIC works with General Practice and other healthcare providers to deliver its vision for “Hillingdon to deliver the best primary care outcomes for patients in the whole of London”. We are a not-for-profit community interest company. The Confederation develops and supports GP practices, PCNs and Neighbourhoods to meet changing needs and to deliver excellent clinical services. We are the provider representative voice for local General Practice into the wider NHS and other Partners, and we are ‘of the NHS’ but independent, innovative and transformational.


Responsibilities

* Undertake home visits in accordance with relevant protocols.
* Make professionally autonomous decisions for which you are accountable.
* Prioritise health problems and intervene in complex, urgent or emergency situations, including initiation of effective emergency care.
* Support patients and their families/care home staff to adopt health promotion strategies and practice self-care.
* Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
* Maintain professional standards in line with the Royal College of Nursing guidance.
* Develop excellent relationships across the PCN to enable collaboration for better patient outcomes; contribute to achieving quality targets.
* Deliver enhanced services and other service requirements on behalf of the PCN.
* Undertake mandatory training and induction programmes.
* Provide effective, visible leadership that promotes professionalism, compassion, excellence and teamwork.
* Maintain up-to-date knowledge of national and local policy and engage in self-development.
* Assess, diagnose, plan, implement and evaluate treatment and care for patients, including those with undifferentiated or complex needs.
* Provide patient-centered clinical care with consideration of physiological and psychological factors.
* Support adherence to prescribed treatments and facilitate patient compliance.
* Support women presenting for family planning, cervical cytology or sexual health, and participate in vaccination and immunisation programmes for adults and children.
* Advise and administer vaccinations as appropriate, including travel vaccines where applicable.


Communication

* Demonstrate sensitive communication, ensuring patients are informed and able to consent to treatment.
* Support patients and carers, recognising diverse communication needs and cultural backgrounds.
* Maintain effective communication within the practice and with external stakeholders; advocate for patients and colleagues.
* Provide information in accessible formats and refer to sources of support (e.g. PALS) where appropriate.


Quality and Professional Practice

* Work within your competence and professional code of conduct as regulated by the NMC.
* Produce accurate and complete patient records in line with legislation and procedures.
* Prioritise and manage workload to maintain quality of care.
* Deliver care in accordance with NSF, NICE guidelines and evidence-based practice.
* Participate in quality governance, audits, evaluations and learning across the practice and wider organisation.
* Lead on the delivery of services or projects as agreed with practice management; contribute to team performance and appraisal.
* Apply legal issues related to safeguarding and vulnerable patients; promote policies on domestic violence, vulnerable adults, substance misuse and addictive behaviour with appropriate referrals.


Team Working and Leadership

* Understand your role and how it develops over time; work effectively as part of a multidisciplinary team.
* Delegate appropriately and establish clear referral mechanisms.
* Provide mentorship and support to junior staff and share best practice.
* Collaborate with the team to define values, direction and policies impacting care delivery.


Risk, Information and Safety

* Manage risk within your areas of responsibility and ensure safe practices and compliance with health, safety and security legislation.
* Oversee safe storage, rotation and disposal of vaccines and drugs; monitor controlled drug usage as applicable.
* Ensure mandatory training for nursing staff is completed; apply infection control measures; promote policies to reduce health risks and improve access to care.
* Use technology to plan, implement and monitor care; manage data with accurate coding and reporting for monitoring and audit.
* Adhere to information governance, confidentiality and data protection requirements.


Learning and Development

* Mentor more junior staff and disseminate learning across the team.
* Identify learning needs, undertake appropriate development, and educate patients and carers to promote understanding.


Health, Equality and Confidentiality

* Comply with local Health and Safety policies and reporting systems; comply with data protection and information governance requirements.
* Promote equality, diversity and inclusive practices; treat all patients and staff with respect and ensure access to care for marginalised groups.
* Uphold patient confidentiality and only disclose information as authorised and necessary for the role.


Requirements and Desirable Skills

* Essential: Registered with the NMC and revalidation; advanced clinical practice skills; leadership, communication, and clinical governance capabilities; experience in nurse-led care of long-term conditions; full driving licence.
* Desirable: Mentor/teaching qualification; knowledge of public health issues; experience working with community development initiatives and health needs assessment.
