Community Case Manager - Nurse Southern ACHT
We have an enviable opportunity for a Community Case Manager in the Southern Adult Community Healthcare Team. Your leadership skills will help the organization reach its goals in providing gold-standard patient care and allow you to contribute to improvements in community care. You will have the opportunity to implement strategies to support the team in delivering efficient and safe care to clients, working closely with colleagues to coordinate visits and managing the administration of holistic care in the community.

Main Duties
The case management model will provide interventions tailored to individual needs.
Contribute to processes for proactively identifying high-intensity users of community and primary healthcare, and those at high risk of unplanned hospital admission.
Assess physical and psychosocial needs, initiating therapeutic treatments and care based on the best available evidence.
Use specialist nursing skills to improve health outcomes.
Manage referrals to develop and establish case management programs for patients referred to the Adult Community Healthcare Teams.
Manage caseloads of patients with long-term conditions benefiting from case management.
Plan, coordinate, and evaluate care programs using the single assessment process, ensuring patients are treated by the most appropriate team member.
Develop protocols and procedures for clinical pathways.

Education, Qualifications & Training
* Registered Allied Healthcare Professional / Registered Level 1 Nurse
* Degree/Diploma in Health/Nursing Studies or related field
* Recent CPD evidence
* Relevant postgraduate clinical training (e.g., Elderly, Long Term Conditions, Respiratory, Cardiac, Dementia, Rehabilitation, Trauma and Orthopaedics, End of Life)
Skills, Abilities & Knowledge
* Specialist assessment, analysis, planning, delivery, and evaluation skills
* Motivational skills for staff
* Excellent communication skills
* Case management of complex cases
* Partnership working
* Knowledge of legal requirements for working with vulnerable adults and those lacking capacity
* Understanding of NHS and social care modernization
* Report writing
* Patient, carer, and colleague education
Experience
* Experience across core areas such as Elderly, Orthopaedics, Neurology, Respiratory, Palliative, Long-term conditions
* Experience working within multidisciplinary teams
* Minimum 3 years post-registration community/in-patient experience
