About the Role: This role will include flexible working and fast paced changes. As a support worker, you will play a vital role in providing practical and emotional assistance to individuals in their own homes assessing their needs.
What will a day in the life of a Support worker involve?
Work closely with people, their families and other professionals to create care plans tailored to their needs and goals.
Assist and enabling individuals with activities of daily living, including bathing, dressing, meal preparation medication management, and mobility support.
Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
Monitor and evaluate individuals progress, adjusting care plans as needed to promote optimal recovery and well-being.
Maintain accurate and thorough documentation of interactions, care plans, and progress.
To be a successful Support Worker, what will you need?
Previous experience in a similar care, preferably within a healthcare or social care setting but not essential full training.
Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
Flexibility to work various shifts, including evenings, weekends, and bank holidays.
