Key Responsibilities
* Carry out holistic, strengths-based assessments of clients' daily living activities.
* Develop and implement tailored care plans to promote independence and improve quality of life.
* Liaise with professionals, agencies, and partners to ensure comprehensive support for each client.
* Recommend and implement equipment and adaptation solutions to meet identified needs.
* Prioritise and manage a varied caseload effectively, balancing urgency and complexity.
* Maintain accurate, detailed records using IT systems such as Mosaic.
* Participate in supervision and professional development activities, ensuring compliance with legislation and safeguarding principles.
* Travel within the community to carry out home visits and provide support in clients' own environments.
What We're Looking For
* At least two years of experience in a relevant setting, with a strong understanding of person-centred care.
* Experience supporting individuals with physical or learning disabilities or mental health conditions is essential.
* A car driver's licence is highly desirable to support home visits across the area.
