Job Opportunity: Support Worker in Intermediate Care
An exciting opportunity has arisen for an experienced Support Worker (RSW) to join our Intermediate Care Team within the Integrated Community & Support Services (ICASS) in the St Andrews Locality.
The Intermediate Care Service provides rehabilitation for adults in the locality, delivered in hospitals, outpatient settings, or patients' homes.
The primary role involves working with Occupational Therapy, Physiotherapy, and District Nursing Services to deliver therapeutic programmes, aiming to prevent hospital admissions and support early discharges. You will carry out daily care functions in the community, primarily dealing with older adults, as part of a multidisciplinary team.
Requirements include an SVQ Level III in Health and Social Care or equivalent. Experience as a support worker and working with older adults is desirable. You should have good organisational skills, excellent communication, and the ability to work independently and within a team. Flexibility to work 15 hours over 7 days and travel throughout the area is essential.
