The CNS role is a vital part of the MDT approach to managing and supporting patients presenting with acute stroke syndromes, as well as the chronic and lasting effects of stroke. The CNS is also involved in the thrombolysis/thrombectomy pathway and is a leader in providing secondary prevention education to patients and their families. You will also run clinics in conjunction with the B7 Stroke CNS and support the identification of AF using the Apoplex system, counselling patients after their week of cardiac monitoring.
In this role, you will contribute to the provision of specialist nursing for stroke patients, practicing within the multi-disciplinary team, providing nursing advice to support and facilitate high quality, individualised care to patients/clients with a diagnosis of a stroke or Transient Ischaemic Attack (TIA) and relatives/carers in collaboration with the CNS team leader and the multi-disciplinary team, in line with local and national targets.This role will allow you to develop wider specialist knowledge and skills working in close collaboration with the other Stroke specialist practitioners within the team.Main duties of the job- Ensure high standards of patient care at all times; challenging and ensuring others challenge poor practise, in accordance with the NMC Professional Code of Conduct.- To assist the Neurology/Stroke team in assessing patients diagnosed with a TIA/stroke, initiating and providing secondary health prevention. - To respond to stroke emergency calls and deliver high dependency nursing, rapid intervention and assessment for all possible strokes.- To be active in the on-call stroke nurse monthly rota, attending emergency stroke calls (thrombolysis and/or thrombectomy) on a rotational basis.- With senior staff, identify and be actively involved in staff education and development, including utilising appropriate learning opportunities.- To act as a mentor, supervisor and resource for student nurses, qualified and unqualified members of the nursing team.- To be actively involved in the development of Trust policies and procedures, providing specialist input, leading their implementation and reinforce their use.- To take the lead in ensuring compliance to Trust policies and procedures amongst the multi-disciplinary team.- To be aware of service delivery plans, participating in planning for future activity and identifying outcome measures of care.Working for our organisationAt Imperial College Healthcare you can achieve extraordinary things with extraordinary people, working with leading clinicians pushing boundaries in patient care.
