What you will do: * Assess and treat adult patients with neurological conditions, with a focus on intervention for acquired communication and swallowing difficulties in the community setting * Provide support to patients in their homes, care homes, and other community settings alongside Sutton GPs * Manage patients with conditions like stroke, head injury, Parkinson's disease, multiple sclerosis, and other progressive conditions * Work autonomously in managing dysphagia, ensuring comprehensive care delivery * Collaborate effectively within the Multidisciplinary Team (MDT) to facilitate holistic patient care * Complete community visits around the borough of Sutton, ensuring the best possible support for patients * Regularly document assessments and interventions using EMIS, provided you have a NHS SMART card Essential requirements: * Proven experience working with patients with neurological conditions * Experience in rehabilitation and managing long-term conditions * Dysphagia trained with the ability to work autonomously * Community experience with adults and older adults * Proficiency in MDT working * Must have a car and a valid driving licence Desirable: * EMIS trained * Able to claim mileage, although parking permits are not available To be a part of this dynamic and fulfilling role, you must be HCPC registered and able to navigate the vibrant community of Carshalton in your own vehicle.
