Main duties of the job

The Practice Nurse will be responsible for delivering high-quality nursing care to patients within a primary care setting. Key duties include running nurse-led clinics, supporting patients with long-term condition management (such as diabetes, asthma, and hypertension), and providing routine nursing services including immunisations, cervical screening, wound care, and health checks.

You will work collaboratively with GPs and the wider clinical team to ensure safe, effective, and patient-centred care. The role also involves promoting health and wellbeing, supporting patients with lifestyle advice, and maintaining accurate clinical records in line with practice protocols.

The successful candidate will contribute to maintaining high clinical standards, infection control, and continuous service improvement within the practice.

Qualifications

* Qualifications:
* Registered Nurse (RGN/RN) with current NMC registration
* Evidence of ongoing professional development
* Experience:
* Experience in primary care, general practice, or community nursing preferred
* Experience in managing long-term conditions (e.g., diabetes, asthma, hypertension)
* Experience delivering immunisations, cervical screening, and wound care
* Skills / Additional Criteria:
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
* Strong organisational and time-management skills
* Compassionate, patient-focused approach
* Competent in maintaining accurate clinical records and adhering to confidentiality standards
* Experience with nurse-led clinics and chronic disease management
* Knowledge of clinical governance and quality improvement processes
* Ability to support health promotion and lifestyle advice initiatives
* Flexibility to work across various clinics or extended hours if required


Experience

* Registered Nurse (RGN/RN) with current NMC registration
* Experience of working within a nursing team in a healthcare setting
* Experience of dealing with vulnerable patients
* Experience providing routine nursing care, including immunisations, wound care, and cervical screening
* Experience managing long-term conditions such as diabetes, asthma, or hypertension
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a multidisciplinary team
* Competent in maintaining accurate clinical records and adhering to confidentiality standards
* Commitment to professional development and keeping up to date with clinical best practice
* Experience in nurse-led clinics or chronic disease management programs
* Experience with health promotion, lifestyle advice, and preventative care initiatives
* Knowledge of clinical governance, quality improvement, and audit processes
* Experience using electronic patient record systems (e.g., EMIS, SystmOne)
* Flexibility to work across different clinics or extended hours if required
* Additional nursing qualifications or certifications relevant to primary care (e.g., immunisation certificate, diabetes care, respiratory care)
