We are looking for individuals who can be part of our fast-moving, innovative, and ambitious teams, dedicated to supporting our communities in managing their health both at home and in our clinics.
You will be responsible for providing skilled nursing care to patients across the locality and working flexibly with multidisciplinary teams to support patients throughout their journey. Our community teams of Nursing and Allied Health Professionals deliver care to individuals with significant physical health needs in various settings, including clinics, community inpatient units, hospices, and patients' homes, including care homes. Our goal is to enable independence, self-care, and optimal living, while reducing hospital admissions.
Services range from community nursing, including specialist practitioners, to public health and health promotion activities such as school nursing and health visiting. Many specialist community services are designed to support care pathways, including Physiotherapy, Occupational Therapy, Speech and Language Therapy, Dietetics, and Podiatry.
Please note that this post does not meet the pay or skill level required for a Skilled Worker visa. Applicants without prior NHS experience would typically be placed at the bottom of the pay band in line with Agenda for Change. The salary offered is below the minimum salary required for sponsorship for a Skilled Worker or Health & Care visa. Therefore, the Trust cannot sponsor for these visas.

You will need to demonstrate excellent communication and interpersonal skills, and be adaptable, reliable, resourceful, and approachable. You should be organized and confident, capable of working independently in patients' homes, with support always available.
