Job Summary
We have an exciting job opportunity for a Practice Nurse to join our large nursing team.
Closing date: 31 August 2026.
Responsibilities
They will implement processes for effective management of patients with long‑term conditions using evidence‑based practice, including care for elderly and housebound patients. As a senior member of the multidisciplinary team, they will ensure nursing services are delivered effectively to the entitled patient population.
Main duties include:
* Infection prevention and control
* Health promotion
* Chronic disease management
* Well women and well man clinics
* Support in reviewing and delivering clinical policy and procedure
* Routine treatments in the general clinic, such as wound management, ECGs, smears, screenings and Imms
* Provide clinical nursing service, support to other professionals and to patients, including treatment, screening and chronic disease management (QOF) and advice within the professional competence of the post holder.
Experience
* Essential: Working in a primary care environment
* Essential: Experience of working autonomously
* Essential: Experience of chronic disease management
* Essential: Experience of infection prevention and control measures
* Desirable: Working with SystemOne
Qualifications
* Essential: Registered Nurse
* Essential: CPR trained
* Desirable: Post‑graduate diploma or degree (chronic disease management)
* Desirable: Family planning qualification
* Desirable: Nurse prescriber
Knowledge and Skills
* Essential: Wound care/removal of sutures and staples
* Essential: ECGs
* Essential: Venepuncture
* Essential: Imms
* Essential: Womens Health
* Essential: Chronic Disease Management
* Essential: Ability to follow and have knowledge of clinical procedures and policy
* Desirable: Knowledge of SystemOne and Read Coding
* Desirable: Strong IT skills
* Desirable: Effective utilisation of resources
