The Home Health Care Speech Therapist provides speech and language therapy services to patients in their home settings. The therapist works with individuals who have difficulties with speech, language, communication, or swallowing due to various medical conditions, including strokes, traumatic brain injuries, neurological disorders, or developmental delays. The therapist creates individualized treatment plans aimed at improving patients’ functional abilities, including communication skills and swallowing function, while promoting independence and improving quality of life.

Work Location: On the road in assigned territory, including Wakefield and surrounding communities

Compensation : Up to $70 per hour

* Patient Evaluation and Assessment:
o Conduct comprehensive assessments to evaluate patients’ speech, language, cognitive-communication, and swallowing abilities.
o Review medical histories and collaborate with the health care team to understand the patient’s condition and treatment goals.
o Identify speech, language, and swallowing deficits, and assess the impact these issues have on daily living and quality of life.
* Development of Treatment Plans:
o Develop individualized treatment plans based on the results of the assessment, the patient’s goals, and specific needs.
o Set short-term and long-term therapy goals in collaboration with the patient, caregivers, and other health care providers.
o Provide therapy interventions to address issues related to articulation, fluency, voice, language, cognition, and swallowing.
* Speech and Language Therapy:
o Provide speech therapy interventions, including exercises and techniques to improve speech articulation, fluency, comprehension, and language skills.
o Use specialized techniques to address voice disorders, stuttering, and other speech disorders, as well as improve auditory processing and cognitive-communication abilities.
o Conduct therapy to improve the patient’s ability to understand and produce speech, language, and nonverbal communication.
* Swallowing Therapy:
o Assess and treat swallowing disorders (dysphagia) to ensure patients can safely eat and drink without risk of choking or aspiration.
o Provide strategies and exercises to strengthen the muscles involved in swallowing and recommend appropriate modifications to food or drink consistency.
* Cognitive-Communication Therapy:
o Implement strategies to address cognitive communication issues such as memory, attention, problem-solving, and reasoning abilities.
o Help patients with brain injuries or neurological conditions regain the ability to manage cognitive tasks, including planning, organizing, and decision-making.
* Education and Training:
o Educate patients and family members or caregivers on speech and language exercises and strategies to be used between therapy sessions.
o Provide training on communication aids, assistive technology, or strategies to enhance patient communication and independence.
o Advise families and caregivers on the best practices for supporting patients’ recovery and daily communication needs.
* Documentation and Reporting:
o Maintain accurate and up-to-date records of patient assessments, progress, therapy sessions, and treatment outcomes.
o Complete necessary documentation for billing and insurance purposes while adhering to health care regulations and compliance standards.
o Report patient progress to the health care team, including physicians and other therapists, to ensure comprehensive care.
* Collaboration with Health Care Team:
o Work with physicians, nurses, occupational therapists, physical therapists, and other health care providers to ensure holistic patient care and to adjust treatment plans as needed.
o Communicate regularly with the health care team about patient progress and any changes in patient condition that may require modification to the therapy plan.
* Safety and Risk Management:
o Ensure a safe environment for patients by making recommendations for communication tools, swallowing modifications, or home adjustments.
o Provide guidance on preventing aspiration and choking risks, particularly for patients with swallowing difficulties.
Qualifications :

Valid driver’s license required.

* Education:
o A degree in Speech-Language Pathology from an accredited program.
o Valid state licensure as a Speech-Language Pathologist (SLP).
* Experience:
o Prior experience as a speech therapist, particularly in home health care, rehabilitation, or related fields, is preferred.
o Experience working with patients who have neurological disorders, stroke recovery, or swallowing difficulties is an advantage.
* Skills:
o Expertise in speech and language therapy techniques, including those related to swallowing and cognitive communication.
o Strong communication skills, both verbal and written, to effectively educate patients, families, and health care teams.
o Ability to work independently, manage patient caseloads, and develop individualized treatment plans in home settings.
o Proficiency in using electronic health records (EHR) for documentation and communication with health care teams.
o Compassionate and patient-focused, with the ability to motivate patients to participate in therapy and improve their outcomes.
