An opportunity has arisen for dynamic and motivated Band 5 Public Health Staff Nurses to work within the newly integrated 0-19 Children's and Young People Public Health Service based in the Abingdon Locality. The service combines Health Visiting, School Health Nursing, and the Family Nurse Partnership, delivering the Healthy Child Programme (HCP) to support children, young people, and families. Health Visitors, Community Public Health Nurses, and Community Public Health Associates will work with children up to 8 years of age with known concerns or vulnerabilities, providing health-related episodes of care in areas such as behaviour, diet, and sleep. A new four-year health and development review has been introduced to support school transition and address school readiness. The role offers opportunities for professional and leadership development, delivery of community-based health promotion activities, and support for Oxfordshire's Young Parents Pathway alongside Specialist Practitioners.
Main duties of the job
1. Contribute to the safe and effective delivery of the 0-19 service within your caseload, working in partnership with children, families, primary care, schools, and other agencies.
2. Deliver services in homes and community settings, including assessing, planning, implementing, and evaluating family-focused care under delegation.
3. Undertake Healthy Child Programme developmental and transitional assessments.
4. Plan, deliver, and evaluate public health initiatives through community-based activities.
5. Adhere to Safeguarding policies and engage in safeguarding processes under supervision.
6. Develop and maintain interagency networks and partnerships.
