Overview
We have an exciting opportunity to join the Oncology Team as a SACT Navigator within our Chemotherapy Services. We are looking for someone who is self-driven, highly organised, with excellent communication and interpersonal skills, and confident in communicating with a variety of teams. You will work with medical teams, pharmacy, pathology, nursing and patients and their families/carers, and develop strong working relationships with secondary care and community colleagues. Experience in Oncology/Haematology or Chemotherapy is preferred.
Successful applicants will be required to provide an immunisation and vaccination report from their UK occupational health provider or UK GP.
Responsibilities
* Support the Oncology Unit Manager/Unit Co-ordinator by assisting the nursing team in coordinating the Unit's activities to maximise efficiency and prevent delays within the pathway.
* Liaise with nursing and oncology pharmacy staff to co-ordinate the supply of systemic anti-cancer treatments (SACT) to be available for the time of each patient’s appointment.
* Liaise between nursing staff, aseptic pharmacy, and outpatient pharmacy (TTOs) regarding patient attendance and communicate go-ahead or any delays.
* Triaging telephone calls and providing basic advice, support and signposting.
* Refer patients to other appropriate healthcare professionals as required.
* Ensure investigations are booked, and results reviewed and authorised prior to treatment.
* Manage patient information and co-ordinate patient pathways through collaboration with medical secretaries, MDT coordinators, clinical nurse specialists and other departments as required within the Trust.
* Ensure timely booking of follow-up outpatient appointments in conjunction with treatment appointments.
* Support the nursing and pharmacy team to ensure patients receive their supportive medication prior to discharge.
* Refer patients to the community nursing team as directed by the nursing team.
* Data collection and analysis: collect data for the Oncology Unit as requested by the Unit Manager or Lead Nurse.
