Offer description

We are looking for someone to enhance our existing clinical team with expertise in clinical medicines management, face to face and remote structured medication reviews, long term condition management, management of medicines on transfer of care and systems for safer prescribing.

This is an exciting opportunity for the right candidate to work as part of the team and manage quality improvement and clinical audits, as well as some aspects of the Quality and Outcomes framework and local prescribing schemes.

There is also the opportunity to be involved in research.


Main duties of the job

Carry out patient-facing Long Term Condition clinics and medication reviews.

Management of common/minor/self-limiting ailments.

Complete all medication switches and reconciliations with discharge letters.

Fulfill contractual requirements of PCN DES and local prescribing schemes.

Support team members and patients with queries about medicines, signposting to appropriate health professionals.

Work with GP's on repeat prescribing scheme / implement national and local guidance.

Work with GP's on service development.

Ensure medicines safety and quality improvement.

About us

We provide primary care services to approximately 6800 patients from our premises in Padgate, Warrington and our branch surgery, a short distance away in Woolston Neighbourhood Hub. We are a supportive practice with a full complement of Clinical staff which includes three GP Partners, four Salaried GP's, and a GP Assistant/ Health Care Assistant, as well as an established team of patient advisors and administrative staff. We are also supported by a number of ARRS roles who work across all PCN practices. We are a dynamic, progressive, forward-thinking and very friendly team, as well as being a training and research practice. We strive to provide excellent clinical care within a mutually supportive environment for professional development.

This role is part of the additional role reimbursement scheme (ARRS), led by NHS England. ARRS roles are recruited to support East Warrington Primary Care Network which comprises three GP Practices. Each individual Practice employs ARRS staff, however on occasions the Network may experience a short staff supply and therefore to ensure business continuity, you may be asked to work across the other Practices within the PCN to ensure a fair allocation and to ensure business runs smoothly for our patients. This would usually be for a short period of time until PCN business resumes to normal.


Job responsibilities

* See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma). Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking, ensuring they get the best use of their medicines (i.e. medicines optimisation). Make appropriate recommendations for medicine improvement.
* Undertake clinical medication reviews with patients and produce recommendations on prescribing and monitoring.

Management of Common/Minor/Self-limiting Ailments

* Managing caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence. Signposting to community pharmacy and referring to GP's or other healthcare professionals where appropriate.
* Provide patient facing clinics and telephone support for those withquestions, queries and concerns about theirmedicines in the practice.

Medicines Information to Practice Staffand Patients

* Answers relevant medicine-related enquiries from GP's, other practice staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes.

Unplanned Hospital Admissions

* Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews. Put in place changes to reduce the prescribing of these medicines to high risk patient groups.

Management of Medicines at Discharge from Hospital

* Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
* Set up and manage systems to ensure continuity ofmedicines supply to highrisk groups of patients.

Signposting

* Ensure that patients are referred to the appropriatehealthcare professional for the appropriate level ofcare within an appropriate period of time e.g.pathology results, common/minor ailments, acuteconditions, long term condition reviews etc.

Repeat Prescribing

* Produce and implement a practice repeatprescribing policy.
* Manage the repeat prescribing reauthorisationprocess by reviewing patient requests for repeatprescriptions and reviewing medicines reachingreview dates and flagging up those needing areview.
* Ensure patients have appropriate monitoring testsin place when required.

Medicines Quality Improvement

* Undertake clinical audits of prescribing in areasdirected by the GP's, feedback the results andimplement changes in conjunction with the practiceteam.
* Implement changes to medicines that result fromMHRA alerts, product withdrawal and other localand national guidance.

Implementation of Local and National Guidelines and Formulary Recommendations

* Monitor practice prescribing against local standards and make recommendations toGP's for medicines that should be prescribed byhospital doctors (red drugs) or subject to sharedcare (amber drugs).
* Assist practices in seeing and maintaining apractice formulary that is hosted on the practicescomputer system.
* Auditing practices compliance against NICEtechnology assessment guidance.

Service Development

* Contribute pharmaceutical advice for thedevelopment and implementation of new servicesthat have medicinal components (e.g. advice ontreatment pathways and patient informationleaflets).

Information Management

* Analyse, interpret and present medicines data tohighlight issues and risks to support decision-making.

Person Specification


Qualifications

* Mandatory registration with General Pharmaceutical Council
* Specialist knowledge acquired through postgraduate diploma level or
* equivalent training/experience.
* Working towards/intent of gaining independent prescribing qualification.


Experience

* Minimum of 2 years post-qualification experience.
* Experience of working in primary care.
* Experience of working in a GP practice.


Skills and Knowledge

* An appreciation of the nature of general practice, primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* Excellent interpersonal, influencing and negotiating skills.
* Excellent written and verbal communication skills.
* Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
* Able to obtain and analyse complex technical information.
* Able to work under pressure and meet deadlines.
* Work effectively independently and as a team member.
* Demonstrates accountability for delivering professional expertise and direct service provision.
* In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.
* Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g.
* Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to GP's when appropriate.


Other

* Full Driving Licence.
* Current DBS.
* Safeguarding adult and children level three trained.
* BLS certified.
