Band 3 Nursing Rehabilitation Assistant - Home Pathway

7.5 hours per week - Full & Part-time hours considered

We are currently seeking Nursing Rehab Assistants to join our dynamic and friendly team, who are patient-focused and thrive on providing quality care, to work within our Home Pathway Rehabilitation Service.

You would be joining a multi-disciplinary team consisting of Rehabilitation Assistants, Physiotherapists, Occupational Therapists and Social Workers. Your role would be to assist patients with personal care and their activities of daily living, and help them with their rehabilitation.

Proposed interview date week beginning 13th October 2025


Main duties of the job

This role will entail:

* Assisting patient with activities of daily living such as washing and dressing and meal preparation
* Rehabilitation in the form of home exercise plans
* Taking patient observations

This role will be suitable for candidates who have a background in healthcare.

We are not looking for clinical qualifications: we’ll give you all the training you need. Additionally, we’ll give you all the support you need to advance and grow your career.

For this opportunity you will need a full UK driving licence and a vehicle that you are able to use for work purposes.

Detailed job description and main responsibilities

· The post holder is responsible to safeguard children, young people and adults at risk.

· Supervise and assist in the provision of hygiene, and other activities of daily living as directed.

· Supervise/assist patients’ mobility following instructions by members of the multi-disciplinary team.

· Deliver appropriate patient care, as indicated by the qualified staff in accordance with the patient’s care plan and to the required standard.

· Move and handle patients using appropriate techniques safely after training.

· Respect the need for patient privacy and dignity at all times.

· Maintain awareness of one's own limitations at all times.

· Participate in one-to-one goal-oriented therapies directed by the therapy staff. (Practising washing & dressing, meals preparation, supervision or prompting with medication, complete home exercise programs and promote independence)

· Report all adverse signs, symptoms, observations to the therapy staff responsible for the patient.

· Demonstrate basic CPR skills after training.

· Develop rapport with patients and relatives to encourage their interest and participation in activities.

· Document care delivered within a specified time frame using EMIS.


Person specification


Qualifications

* Educated to GCSE or equivalent.
* NVQ level 2, equivalent, or willing to work towards.


Experience

* Working in a busy environment.
* Experience in caring situations.
* Experience working in community rehab setting
* Experience of completing patients obs/vital signs


Training

* Completion of Care Certificate or willing to work towards.


Information Technology

* Experience using smart technology e.g. tablets and phones
* Experience using EMIS/Civica
* Experience using MS Teams


Skills

* Ability to accept appropriate degree of responsibility.
* Ability to remain calm.
* Ability to recognise limitations
* Able to work individually and as part of a team.

Salary is determined by NHS experience and current banding/pay point. External applicants will begin at the entry point of the salary scale in line with NHS terms and conditions.
