An opportunity has arisen for a Senior Social Worker to join the Hospital Discharge Team at Windsor and Maidenhead Council. This full-time locum role offers the chance to make a meaningful difference in people’s lives, ensuring timely and effective hospital discharges while supporting a collaborative and innovative team environment. Candidates must have prior experience in hospital discharge and supervisory responsibilities.
Key Responsibilities:
* Deliver statutory services to individuals with eligible care needs through person-centred and outcome-focused assessments.
* Provide supervision and guidance to team members, supporting their development and maintaining high professional standards.
* Work closely with health and social care professionals to facilitate safe and effective hospital discharges.
* Review, update and monitor care plans to ensure they reflect the needs and wishes of service users.
* Contribute to continuous improvement within the service, fostering innovation and best practice in care delivery.
Candidate Requirements:
* Qualified Social Worker with registration to Social Work England (SWE).
* Supervisory or senior-level experience within social work.
* Strong assessment, care planning and multi-agency working skills.
* Ability to lead by example, motivate others and ensure high-quality service delivery.
