An exciting opportunity has arisen for an experienced nurse to join our well-established team within iSPA Integrated Care Co-ordination Service (ICC).

Our team plays a vital role in triaging urgent referrals from a range of sources, including telephone-based systems and CAD portal referrals from the ambulance service. These often require a two-hour or same-day response for the Urgent Community Response Teams (UCR). We also provide signposting to more appropriate services when needed and can offer clinical advice where this is the most suitable option.

In this role, you will use your clinical expertise to safely triage patients, manage ambulance referrals via the CAD portal, and support access to services across primary, secondary, and out-of-hospital care. You will draw on your strong clinical knowledge and risk assessment skills to distinguish between urgent and non-urgent cases, ensuring patients receive timely and appropriate care within the community setting.

Main duties of the job

The post holder will use their sound clinical knowledge and risk assessment skills to prioritise referrals through the clinical triage service as necessary.

The Post holder will effectively coordinate the resources available within Out of Hospital services to maximise access to services or sign post to other appropriate services available.

The post holder will ensure that all contacts with the clinical triage services reach an effective and appropriate outcome for the patient.


Organisational

To provide managerial support to the team including Performance Development Reviews, Clinical Supervision and Recruitment and Selection

To provide leadership to the clinical team

To provide knowledge and guidance with respect of clinical health services and to ensure and facilitate an effective interface between primary care services and out of hospital services

Contribute towards ensuring the AHP & Nursing Strategy is implemented.

Participate in the development, training and mentorship of pre-registration students/medical students and staff as required including induction and preceptorship programmes

To provide assistance with the resolution of complaints within the clinical speciality, or sphere of responsibility

Qualifications

Essential

* NMC Registered Adult Nurse (1st Level)
* Hold Degree level qualification in related subject
* Evidence of successful completion of a relevant formal post graduate module, e.g. Advanced Health Assessment, Long-term conditions.
* Evidence of ongoing CPD
* Evidence of successful completion of a leadership course, eg. Mary Seacole, LEO

Desirable

* Telephone triage training
* Nurse Prescribing
* Coaching skills


Experience

Essential

* Significant post registration experience in relevant clinical area, e.g. Emergency/Urgent Care, District Nursing.
* Experience of personalised care planning and complex patient management
* Competent end of life experience
* Experience of working in a team environment
* Experience in triage skills

Desirable

* Experience of quality/service improvement work


Skills

* Extensive general knowledge of adult health conditions, wound care principles and current best practices of care.
