Job Overview

We are looking for enthusiastic, skilled and motivated Community Healthcare Support Workers to join our Quickstart service across Wandsworth and Merton boroughs to deliver high quality, timely care support with an enablement focus to patients in the community. We have one permanent position. The Quickstart service provides rapid response social care services for patients at risk of social care breakdown who would otherwise be admitted to hospital due to an overriding social care need. Quickstart works closely with the Maximising Independence Team, a dynamic, multi‑disciplinary team of physiotherapists, occupational therapists, speech and language therapists and rehab assistants, allowing joint working with rehabilitation and learning opportunities. The post offers the successful candidate an opportunity to develop their clinical skills and knowledge base.

Responsibilities

Deliver high quality, timely care support to patients in the community. Work closely with a multi‑disciplinary team to support rehabilitation and learning. Participate in training and supervision provided by senior therapists. Use Trust laptop and mobile phone for mobile, remote working. Actively support patients at risk of social care breakdown to prevent hospital admission.
