Job summary
We are looking to recruit a band 5 Community Learning Disability Nurse who may either already be qualified, or in final year and approaching qualification and/or awaiting their NMC PIN number. You must have enthusiasm, compassion and commitment to supporting and enabling people with Learning Disabilities. You will join an already established multidisciplinary community learning disability team, which is located at Monkton Hall in South Tyneside. You will work primarily within the borough of South Tyneside, however there may be occasions where you will in reach to other areas, for example, with transitions. You caseload will be diverse, of moderate complexity and at times challenging, however you will be supported with clinical support, clinical and management supervision, regular and ad hoc. You will have an experienced supervisor.
You will offer leadership to support staff within the team and will act as a coordinator for clinical cases, ensuring best practice and safe care across a spectrum of need.
You will work Monday to Friday, core Hours being between 08:00-18:00, where you will work 7.5hrs within this.
Main duties of the job
* To manage a delegated clinical caseload of individuals who have a Learning Disabilities within community setting.
* To assess plan, implement and evaluate treatment interventions supported by line manager through supervision.
* To facilitate Health Action Planning using the agreed framework where appropriate.
* To provide and input information onto database in relation to Health Action Planning.
* Provide specialist clinical advice, to other disciplines.
* To communicate effectively and develop effective working relationships with other stakeholders.
* Assist and encourage individuals to access mainstream health care services.
* To manage a delegated caseload of individuals who have complex health issues i.e. challenging behaviour, mental health, complex epilepsy and other related health conditions under the direction and supervision of an experienced Band 6 Community Nurse.
* To have the ability to work as a lone worker, act upon own initiative and organise own workload.
* To treat everyone as an individual in their own right. respecting ethnicity, race, culture, gender, age, religion, language, disability and sexual orientation.
* Administration and monitoring of medications as per Nursing and Midwifery Council guidance in keeping.
