Role Summary

The key areas of responsibility will be to support the patients journey through their hospital stay, and to ensure a smooth transition from admission to discharge, whether that be to their home or to an alternative agreed placement. You will be integral to the wards and supporting patient flow. You will be sharing information on expected discharges and progress of patients continuing journey towards discharge. Escalating any delays in achieving this and working with the Multidisciplinary teams to find a solution.

We are looking for a Discharge Co-ordinator to join our team!

In this varied role you will support and co-ordinate patients discharges in a responsible, safe, timely and pro-active manner. Working in partnership with the Multidisciplinary team (MDT), including nursing, medical, therapy, various social services partners, carers, care agencies, community teams and other external partners.

The post holder will ensure discharges are lined up for the earliest opportunity each day, escalating to the appropriate leads where discharges are at risk of failing. You will also be required to keep up to date records of the patients discharge planning in the patients notes. You will work with the MDT to ensure all patients have up to date, realistic discharge plans and take action to implement, monitor and amend the plans if necessary, in partnership will all relevant colleagues.

You will commence discharge planning discussions with the MDT from day one of admission, or as soon as possible and promote that the Estimated Discharge Date is set meaningfully and realistically in the first instance, reviewed daily and accurately recorded.
