Overview
We are looking for an experienced Practice Nurse to join our friendly primary care nursing team in an established GP surgery in Coningsby, Lincolnshire. This is an exciting opportunity to work in general practice, delivering high-quality nursing care to a registered patient list of 8,000. You will work 21.5 hours per week (flexible working considered) and be a key part of our multidisciplinary team.
Experience in general practice nursing is essential, and knowledge of QOF, Enhanced Services, CQC and NICE guidelines will be key to success in this role.
Responsibilities
* Provide safe, evidence-based nursing care to patients of all ages.
* Carry out baby and childhood immunisations in line with NHS vaccination schedules.
* Manage patients with chronic illnesses such as diabetes, COPD and asthma.
* Undertake long-term condition reviews, including health checks and lifestyle advice.
* Contribute to meeting QOF and Enhanced Services targets.
* Maintain accurate and timely patient records using clinical IT systems (e.g. EMIS).
* Work collaboratively within the practice nursing team, GPs, and other healthcare professionals to provide patient-centred care.

Qualifications
Essential
* NMC Registered Nurse
* Competency in baby immunisations, diabetes, COPD, asthma and long-term condition management
* Proven experience in general practice nursing or primary care nursing
Desirable
* PgCert in Practice Nursing (1st Class)
* Experience working in a GP surgery or community health environment
