What youll be doing: * Supporting individuals with complex needs, including mental health, physical health, and substance misuse issues. * Assessing and managing risks to ensure safety and well-being. * Delivering high-quality care, support, and housing management in line with organisational policies. * Working collaboratively with partner agencies to provide holistic, tailored support. * Developing and implementing client-centred support and risk management plans, aiming for suitable move-on options within six weeks. * Building positive relationships, motivating progress, and encouraging recovery. * Managing challenging situations effectively, ensuring safety, and reporting safeguarding concerns. * Maintaining accurate case records, updating IT systems, and producing reports. * Participating in rota shifts, including evenings, weekends, and bank holidays, to ensure continuous support. * Assisting clients with housing benefit claims and tenancy-related administrative tasks. What youll bring: * Understanding of the needs of homeless people and experience working face-to-face support. * Experience supporting individuals with complex needs. * Knowledge of trauma-informed care and move-on options. * Excellent communication skills, adaptable to diverse clients and stakeholders. * Strong decision-making skills and the ability to respond effectively in challenging situations. * IT proficiency, including Microsoft Office and database management. * Ability to work independently and as part of a team with a proactive attitude. * Relevant qualifications or certifications in health, social care, or related fields are desirable. * Commitment to safeguarding, equality, and diversity.
