You will work alongside service users to promote independence, physical health, and emotional wellbeing, while helping them to build strong support networks and engage with specialist services. Key Responsibilities : - Deliver recovery-focused, person-centred support through individual key work and targeted interventions. - Empower individuals to exercise choice and control over their care, creating tailored recovery plans based on their needs and goals. - Act as a champion for key areas such as psychological education, physical health, and eating disorders, sharing expertise and developing resources within the team. - Foster relationships with local services and agencies to ensure individuals receive appropriate support and community engagement. - Ensure accurate record-keeping and documentation, maintaining confidentiality and using IT systems effectively.
