Overview
Two Clinical Pharmacists are sought to join the North Solihull Primary Care Network (PCN). Full-time and part-time considered. You will contribute to improving patient care, supporting GP practices, and optimising medicines use across six GP Practices serving a population of 40,000.
As a Clinical Pharmacist within our PCN, you will work across GP practices with a multidisciplinary team and be involved in managing long-term conditions, carrying out structured medication reviews, and supporting safe prescribing. There is the opportunity to run pharmacist-led clinics and, where appropriate, manage minor illnesses.
Responsibilities
* Work as a clinical pharmacist within the PCN, supported by a GP lead for development and mentorship. Travel between allocated surgeries to ensure equitable access to services.
* Collaborate as part of a multidisciplinary team (GPs, nurses, HCAs, physiotherapists, paramedics, social prescribing link workers, Health and Wellbeing Coaches, administrative staff) to provide holistic care and medicines expertise.
* Take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to manage polypharmacy and complex cases. Areas of focus may include CVD, COPD, asthma, diabetes, frailty, etc.
* Provide primary support to practice staff for prescription and medication queries (face-to-face, home visits, care homes, telephone or online). Support repeat prescription systems, process acute requests, medicines reconciliation on transfer of care, and implement safer prescribing practices.
* Run minor ailment clinics in surgery as appropriate for a pharmacy-led service.
* Provide clinical leadership on medicines optimisation and quality improvement; manage aspects of the Quality and Outcomes Framework and enhanced services.
* Ensure integration with community and hospital pharmacy to optimise skill mix, improve patient outcomes, and manage workload.
* Contribute to improving quality of care and operational efficiency; may be supported to become a non-medical prescriber.

Person Specification
Professional Registration
* Essential: Mandatory registration with General Pharmaceutical Council
Desirable: Membership of the Royal Pharmaceutical Society; member of or working towards Faculty membership of the Royal Pharmaceutical Society
Qualifications
* Essential: Masters degree in pharmacy (MPharm) or equivalent
Desirable: Specialist knowledge via postgraduate diploma or equivalent; independent prescriber or working towards independent prescribing qualification

* Essential: Safeguarding adult and children level three
Desirable: Full UK Driving Licence
Skills, Knowledge and Experience
* Essential: Minimum of 2 years post-qualification experience
Desirable: Excellent written and verbal communication skills; ability to explain complex information to diverse audiences
