Band 6 Lead Nurse – coordinated care for a defined caseload of service users within the AOT team.


Responsibilities

* Coordinate care and support for identified caseloads of service users.
* Maintain NMC registration and adhere to professional guidelines and code of conduct.
* Work within the framework of relevant local and national policy and legislation.
* Provide supervision to junior members of the staff team.
* Plan, implement and evaluate nursing care within agreed parameters and maintain a working knowledge of local and national policies.
* Assess and screen patients referred to the AOT service.
* Ensure effective communication pathways with internal and external services.
* Promote equality, diversity and rights in accordance with good practice and legislation.
* Identify and contribute to training needs for self and supervised staff to deliver quality evidence‑based care.
* Support colleagues in the effective assessment and delivery of interventions to individuals within their assigned caseload.
* Undertake comprehensive assessments of an individual’s health and social care needs in a variety of settings and agree the most appropriate course of action.
* Assess, evaluate and manage clinical risk and alert senior staff when appropriate.
* Formulate a plan of care based on identified needs.
* Deliver evidence‑based interventions as appropriate.
* Review effectiveness of care plans at regular intervals.
* Work collaboratively with service users, carers and partner agencies.
* Maintain accurate computer‑held care records.
* Work in a culturally sensitive manner.
* Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways.
* Develop effective working relationships with Primary Care colleagues.
* Act as Team Manager in the absence of the Team Manager.


Professional Duties

* Participate in the Trust appraisal process and lead on professional development, mandatory training and supervisory needs.
* Access and participate in clinical supervision.
* Safely manage and administer medicines as defined by Trust policy.
* Contribute actively to research, service development and clinical governance.
* Ensure confidentiality in accordance with the Data Protection Act, Trust policy and best practice.
* Participate in joint working with appropriate services and agencies.
* Maintain a professional manner when interacting with patients, carers and colleagues.


Supervision Responsibilities

* Regularly review own caseload with a supervisor to promote efficacy and overall service delivery.
* Deliver supervision and facilitate PDR with junior staff.
* Participate in own PDR process, identify training needs and pursue relevant actions to maintain and improve competence.
