Key responsibilities
- Conducting comprehensive assessments of individuals’ needs and circumstances to inform care planning.
- Collaborating with healthcare professionals, families, and community organizations to facilitate effective discharge planning.
- Implementing and reviewing care plans, ensuring they are tailored to the individual’s requirements.
- Providing ongoing support and guidance to individuals and their families throughout the transition process.
- Maintaining accurate and up‑to‑date records and documentation in accordance with organizational policies and procedures.
Qualifications and skills
- A recognized social work qualification (e.g., BA/MA in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Experience working in adult social care, particularly within a hospital or D2A setting.
- Strong assessment and analytical skills, with the ability to make informed decisions.
