Position Overview
The role of a Health Visitor is to promote good health and prevent ill health in children, families and communities. Health Visitors raise awareness of and identify unmet health needs, initiate, develop, implement and evaluate plans of care in partnership with families, primary care, statutory and voluntary services and other partner agencies. They work in partnership to facilitate health‑enhancing activities, build networks within the community, inform and implement policies that affect health outcomes and deliver the Healthy Child Programme. The role is to act as a named Health Visitor within a zone of a corporate caseload and to be a link Health Visitor for identified GP practices. This service is delivered within Cambridgeshire.
Key Responsibilities
* Deliver child and family focused service delivery that makes Every Contact Count.
* Work within an integrated 0‑19 HCP skill mix team.
* Provide a holistic approach to offering support and guidance via management, safeguarding and restorative supervision.
* Participate in continuous staff development identified via supervision and appraisal to support and develop leadership skills.
* Work for an NHS organisation with Agenda for Change terms and conditions.
* Build relationships with partner agencies to ensure the best outcomes for children and families.
