An exciting opportunity has arisen for a Senior Nurse Practitioner (SNP) to be part of our Hastings and Rother Neighbourhood Mental Health Teams (NMHT). You will be part of the clinical leadership team and take lead with our Triage Team, ensuring the safe delivery and management of referrals into NMHT.
Key Responsibilities
* Triage and manage referrals into the NMHT within agreed time‑scales and appropriate clinical need.
* Demonstrate excellent assessment skills and knowledge of mental health services and third‑sector organisations.
* Work with the medical team to ensure effective patient flow and discharge management.
* Provide strong clinical leadership and expertise to the multidisciplinary team and NMHT, maintaining high standards.
* Build trusting relationships with families, carers, local partners and the wider NMHT.
* Work flexibly within multidisciplinary and multiprofessional teams across a range of settings.
Professional Qualifications and Experience
* Substantial clinical experience within a community mental health setting.
* Leadership acumen and ability to collaborate across a range of partners.
* Strong teamwork skills, initiative, creativity, flexibility, a good sense of humour, and the ability to engage people to bring out their best.
