Community Staff Nurse – Community Nursing

We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.
The shift pattern includes weekdays, weekends, out of hours and public holidays.
Responsibilities
The role of Community Staff Nurse will be responsible for the support and management of a caseload of community patients, working in partnership with the District Nurse to assist in leading and managing a team of qualified nursing staff and health support workers. You will act as an independent autonomous practitioner, using in-depth knowledge, expertise, proficiency and experience to provide a high quality nursing service.
You will be responsible and accountable for the ongoing management of patient care, ensuring assessment, planning, implementation and evaluation of programmes of care. These will be developed in partnership with patients, families, carers, and significant others, and will be evidence‑based considering lifestyle, race, gender, cultural background and factors impacting health.
Qualifications & Requirements
* Registered Nurse with the NMC PIN up to date.
* Member of Disclosure Scotland's PVG.
* Driving licence essential and access to a car required; department North West locality.
Other Details
Salary is pro‑rata for part‑time hours.
