Exciting 12‑month secondment Band 6 Specialist Staff Nurse/Trusted Assessor to join the Care Point Team at North Tyneside General Hospital. Focus on trusted assessor / intermediate care element to support patient flow to care homes in North Tyneside.
Responsibilities
* Provide high quality nursing care to adults and support patient discharge pathways.
* Work across sites and collaborate with nursing, social care, OT and physio teams.
* Assess, plan, implement and evaluate care within an environment of informed consent.
* Assist with discharge planning, complex discharges and community referrals.
* Attend ward MDTs, support complex discharges and provide guidance on community services.
* Carry out urgent assessment in patient homes and work within the community setting.
* Model compassionate and inclusive leadership and promote team working across organisational boundaries.
Qualifications & Requirements
* Registered Nurse.
* Experience in community nursing and knowledge of the Care Act and safeguarding procedures (advantage).
* Excellent communication skills and ability to work independently and as part of a team.
* Meeting transport requirements of the post.
* Demonstrated ongoing professional development and ability to meet the fit and proper person requirements.
