Job overview
This role is offered on a secondment basis for 12 months. The hours are part‑time, 15 hours per week. The main focus is delivering a Vocational Rehabilitation service within Berkeley Vale PCN. Patients will be assessed and suitable advice and recommendations will be provided, including the appropriate issue of Med 3 Fit notes, recommendations on reasonable adjustments to employment, and referral to other MDT members.
Main duties of the job
* Run clinics within the primary care network where identified patients are assessed and outcomes measured.
* Create individual treatment plans including the issue of Med 3 Fit notes and fitness to work reports when appropriate.
* Advise and work with patients to implement reasonable adjustments to their employed role.
* Measure outcomes using the Work and Social Adjustment Scale.
* Record treatment interventions following Trust and PCN procedures and maintain HCPC standards of practice.
* Communicate with other members of the MDT including employment specialists, GPs and other professionals.
* Maintain accurate and up‑to‑date data on all interventions to feed into the end‑of‑project report and local dashboards.
Person specification – Qualifications
* Degree or Diploma in Occupational Therapy.
* Evidence of relevant postgraduate specialist training or equivalent experience and short courses to MSc, postgraduate diploma level or equivalent.
* Experience of vocational rehabilitation.
* Experience of working in Primary Care.
Professional and specialist knowledge
* Robust knowledge of governance issues including consent, person‑centred planning, health facilitation, care programme approach and data protection.
* Experience of leadership of staff.
