Wolverhampton Health Visiting Service is undergoing exciting development and expansion, resulting in the creation of new posts. We are seeking an enthusiastic and dynamic SEND Health Visitor to join our 0-19 Health Visiting Service.

Are you an inspirational and highly driven experienced or newly qualified Health Visitor with exceptional leadership and clinical skills, eager to make a difference to our SEND health visiting workforce and families in Wolverhampton?

Car owner/driver essential; you must meet the travel requirements of the post.


Main duties of the job

1. Understand local needs and priorities regarding SEND (special educational needs and disability) in children under 5 and influence HCP (Healthy Child Programme) provision accordingly.
2. Deliver child-centered care to children under 5 with cognitive, social, emotional difficulties, physical disabilities, or sensory impairments.
3. Provide support and guidance to nurseries, educational settings, and other professionals on designated caseloads.
4. Collaborate with the Health Visiting Team, IASS, and partners in education, including DMO and DCO.
5. Facilitate transitions for children and young people entering or leaving caseloads.
6. Support the CDC through attendance at monthly panels and liaise with professionals and services as needed.
7. Work with families, community groups, and professionals to ensure consistent advice and delivery of services to improve health and wellbeing.

Detailed job description and main responsibilities

1. Manage and deliver care programs for a designated caseload.
2. Ensure clinically effective practice using evidence-based standards and policies.
3. Promote continuous quality improvement through audits, feedback, and reflection.
4. Identify health needs of families and communities using appropriate tools.
5. Maintain a safe environment, ensuring compliance with legislation and policies.
6. Communicate effectively with multi-professional teams and partners.
7. Maintain high standards of record-keeping and report incidents promptly.
8. Plan and coordinate care episodes, ensuring smooth transitions and effective communication.
9. Identify and participate in safeguarding activities for vulnerable families and children.
10. Assess, deliver, and evaluate health interventions, influencing clinical decisions.
11. Develop a family-centered care culture within multidisciplinary teams.
12. Engage with children and families regularly, respecting diversity and promoting partnership in care.
13. Identify opportunities to develop services and promote health education.
14. Utilize public health data to address local needs and build community capacity.
15. Lead by example, develop staff, and influence care practices.
16. Participate in appraisals, training, and professional development.
17. Support recruitment, attendance, and performance management.
18. Manage caseloads and delegate appropriately to support staff.
19. Network with peers, attend forums, and promote service excellence.
20. Support service redesign and organizational priorities.
21. Ensure compliance with quality and safety standards, supporting the 'Always Events' initiative.

* NMC Registration


Experience

* Experience as a Health Visitor
* Strong communication skills
