The shift pattern for this post is Monday- Friday 9am-5pm, however service review may require changes in hours.
Responsibilities
An exciting post has arisen with the Care Home Nursing Team within Glasgow City HSCP, the largest of our six HSCPs. The post offers partnership working alongside other specialist health professionals, commissioning and social work to support Glasgow Care Homes. The role includes providing clinical expertise to support improvements aligned to the care inspectorate health and social care standards and care assurance outcomes. The 6 key priorities around nursing care intervention include Infection Prevention Control, tissue viability, palliative care, person centred care, food, fluid and nutrition and falls prevention. A pinnacle part of this role is carrying out care assurance visits to all 93 care homes that are registered within Glasgow City HSCP. The role includes the opportunity to support the practice development nurses in the delivery of training and education to care home staff and managers. The post holder will provide/deliver integrated, collaborative and co-ordinated services to support the care of nursing, residential, adult residents as well as care home staff/managers in order to support high quality standards of care. The post holder will work closely with Commissioning and Social work Services.
There will be an emphasis on professional development and support to care Homes to deliver clinically effective services in line with governance and organisational development strategies of the service. The post holder requires to be a 1st level registered nurse, minimum of 2 years post registration experience with evidence of professional development. Experience and knowledge of the care home sector would be an advantage for this post. Can demonstrate an expert level of knowledge across nursing practice and the ability to implement improvements that may challenge existing practices.
